Meet Alison, our Personalised Care Team Lead

 

Alison Morgan is the recently appointed Personalised Care Team Lead at St Neots Primary Care Network. She heads a team of care coordinators and social prescribers who give nonclinical support to patients.

The team comprises Alison and fellow social prescribers, Petra and Lucy, plus care coordinators Julie and Louise. As well as their regular duties they are also working on a project to give additional support to frail and elderly patients who frequently get in touch with their GP practice. The team also do cancer care reviews and will also start dementia care reviews as well as building links with local and national community groups, charities and stakeholders.

Alison has an extensive background in healthcare and is used to leading teams and prioritising work: 'Generally, in whichever role or project our aim is to look after patients, listen to what they need and put together a personalised care and support plan or suggest activities, strategies and signposting to improve their lives. The more we can do the better the outcome for our patients.

With social prescribing we can offer our patients 12 appointments over a year or 18 months to give continuity of care. I have always treasured that contact which builds relationships and trust.  Although I am new in the role I would rather hit the ground running because it keeps me on my toes and I love it here. Everyone is really helpful and I couldn't do my job without their support.'
