Social prescribers take part in community event
The Four Counties Social Prescribers, Jo and Lucy, took part in a 'Connected Communities' event where they networked with other professionals from the area offering health and wellbeing services.
Jo and Lucy work with Lakeside patients at Stamford and Bourne, offering personalised care and support depending on individual needs. They had a stand at the Connected Communities event where representatives from agencies, charities and care organisations met and found ways of linking up services in order to benefit their patients and clients.
Lucy said:
'It was a great opportunity for us all to get together, hear a bit more about each other's work and explore more integrated ways of working. It was great to meet people that perhaps you only knew through email contact before and there are plans to have another similar event that is open to the public.'
