The Health Outreach Support Team (HOST) includes:
The HOST event provides a 'one-stop shop' in a safe and supportive environment, allowing clients to access the bespoke services they need.
It takes place on Wednesday 26 October 2022 at Cottingham Road Surgery between 10am and 3pm and dedicated rooms will be allocated to different practitioners.
Lead Social Prescribing Link Worker, Kareema Macarthy, said:
'This is a real team effort from a variety of agencies and organisations to help the homeless and those who may be sleeping rough or sofa surfing. It is a way for them to access advice and services at one time in a town centre location. The Lakeside Healthcare team is working closely with partners from North Northants Council, Citizen's Advice, the Department for Work and Pensions, Bridge and Rockingham Forest Primary Care Network to identify and address the needs of this specific group within our community. People can drop in if they need help or advice and the Rough Sleeping Team will be encouraging their known contacts to access the range of services.'
